Moderate Cystocele/Frequent UTIs

I have a moderate cystocele (bladder) and within the past year, I have frequent (what feels like) UTIs.
There irritation is there and the need to urinate followed is followed by that "pinchy pain".

I've mastered the art of cranberry tabs, Cystex (cran tabs with an antibacterial ingredient) straight cranberry juice, baking soda and water, lots of water, eliminating caffeine and alcohol...and this particular time...I nipped it in about 24 hours. After the "pinchy pain" passes...the bladder just feels a little sore...but within a few days after that, I feel back to normal.

My question...is this really a UTI?...As I do not feel as if I have an "infection"...But in the past, I called in for an antibiotic and took it...or is it just symptomatic of the issues with the cystocele...of not being able to fully empty my bladder.

Is it necessary to take an antibiotic every time this occurs?
I've been researching and read that small doses of antibiotics can be taken daily but that's not something I want to do...especially if these other methods are working.

I guess to put it more simply...Can one have UTI symptoms without actually having an "infection".

I have an urethrocystocele and on bad days my bladder feels sore. I just feel the bladder every second but I have no pain while peeing. So it is possible to have this sore feeling without having an infection.
I know this is not very helpful. Just want to tell you my experience.

Just wondering if your urethral area or the entrance to the urethra is in contact with your undies? I think it is possible that the urethra is being contaminated or 'burnt' by traces of urine or faeces on your undies, and maybe chafed as well. I suggest that you keep your crotch area as free as possible of contact with undies and see if it makes a difference in a few days, doing the things you described as well. You might be able to go without undies; maybe wear a slip instead under skirts or dresses. Or wear baggy undies that will droop lower in the crotch. You could also change undies or panty liner several times a day, ensure that only clean cotton is against your vulva. Don't use perfumed panty liners if you have to use them at all (less ventilation). Avoid friction near your urethra during sex. Use only a little pure soap to wash undies, and rinse very well, and dry them in the sun. A little vinegar added to the rinse water is antibacterial and more friendly to good microflora, because soap may leave undies alkaline. Try leaving your pubic hair long. This will give some assistance with your undies 'sliding' over your vulva, and put a barrier between your vulva and undies.

Yes, not being able to empty fully is a symptom of cystocele. When there is cystocele it can be like trying to drain the last of the tea out of the teapot. Unless you can tip the bladder far enough forward during urination the last bit stays in there because the urethra is not at the bottom of the bladder. It can leave stale urine in your bladder, which makes it easier for bugs to colonise your bladder. Not drinking enough water every day will also concentrate your urine, and will mean that your bladder doesn't get flushed out thoroughly very often. It won't feel like the pinchy pain of UTI but it might irritate a bit on the way out if your urine is more concentrated than normal. It becomes a bit like nappy rash, with concentrated urine passing over and burning an area of tissue that is already irritated, whether from the last urination or chafing of undies or the acidic combination of urine hitting faecal smears on undies.

Any hygiene measures, in the way of cutting down cross contamination from the anus, will help break a vicious cycle of irritation. The bladder and urethra are pretty good at preventing full scale infection but sometimes they need a little help.
