So how does the urethra close?

This question is about incontinence. When we cough or sneeze how does the urethra keep us continent? Is it because the external sphincter increases closer automatically (due to intraabdominal pressure), or is it because the pressure from above squashes it closed?
(sorry I was reading about this in one of Christine's replies and now I'm confused)
Thanks & kind wishes to all xxx

The Posture and the Pessary wearer

I have today downloaded the Posture and have of course done a dry run. Sure I'll get the hang of it sooner or later but perhaps I am not a candidate? Can these be done while using the pessary. Also, have a metal plate at lumbar 4&5 put in after broken back 16 years ago. Perhaps Christine could tell me whether these situations are incompatible with the posture.I am not able to sit on the floor to do the sitting excercises due to spinal cord injury resulting in neurological damage to my right leg (walk with a cane)But I am a very positive person and will try anything to improve my quality of life, as long as it isn't a pill.

Trimo-San

This is my first contribution, and am really seeking more info. on alternative vaginal jellies to use with a pessary (which came incidentally with Trimo-San). I was given instant relief (after only two fittings) two weeks ago, of the pain, discomfort and pressure of prolapsed bladder. I have had 5 children and at my age (75) did not want to have surgery. When I contemplated a trip, and thought about additional supplies of Trimo-San I might need, I went on line and lierally stumbled on Christine's article! Horrified wasn't the word! I immediately stopped using it; I too had a cheesey discharge, which I hope will now stop.

Herbal solution - q for Christine

Dear Christine. You made reference a couple of weeks ago to a herbal solution which you put on cotton and insert vaginally. I can't find the thread to put my question on that. I'd be grateful to find out is the herbal solution one you make or buy over the counter? I was very surprised that no-one else asked about it - perhaps I'm one of the few (browsing here for a year now) that are still hoping for a quick fix, despite knowing there isn't one! Many thanks.

Hi there Christine

Where can I get hold of your book, I live in South Africa?

Pregnant with severe prolapse?

I am a new member, though I have been reading on this site for several months now. It has been very helpful. I have a severe uterine prolapse, rectocele, and cystocele (I don't know the degrees, but I do know that my uterus is out of my body and my obgyn said that my case is as bad as it gets - ugh!). I am choosing not to have surgery at this point, and I know I need to start doing the posture and exercises that I've been reading about here (though I wonder if they will do me any good as my case seems extreme?!). My question is, if I got pregant again (we have four children) would everything be all right? It seems hard to imagine the uterus being able to move back up where it needs to be during pregnancy.

advice badly neede!!

hi, im new to this so bear with me please.
i have two teenage kids, youngest is 16, i was aware after that birth that i had changed down below, i wasnt stitched at all and the birth weight was over 10lbs.
i also noticed the difference during sex as i felt looser, this has slowly gnawed at my confidence.
after many years i went to a gyn at the family planning clinic and told her my concerns, basically i was told this is to be expeceted.
i have since the age of 35 suffered mild leakage and do my kegels every day.
earlier in the year i discovered a lump at the entrance to my vagina, i was in shock!!, gp told me i have a cystocele?, is that the correct spelling??, anyway i had a poke around (sorry for any visuals!!) and the lump seems to be like a pad of fat between my bladder and vagina, could doing kegels have enlarged this thing?, also i noticed round about it feels torn so it just kinda hangs there.

I can't be fitted with a pessary!

I finally saw the urologist yesterday, after waiting 6 weeks. I'm finally, officially diagnosed, after having the condition for 15 years! LOL! She was completely sympathetic to my wish for non-surgical methods of dealing with the situation, and made an appointment with the nurse practioner for later in the afternoon to get a pessary fitted. I thought one might be helpful in lifting things without causing myself more damage.

We tried several different pessaries, but each time, when the NP sent me to the toilet to try sitting down, when I walked back, the thing was sticking out part of the way! Even when she upped it to the humongous doughnut shaped one that hurt to go in-- the darn thing was sticking out when I came back into the exam room. And they do hurt when doing that!

Limitations?

I’m wondering what sorts of things prolapsed women think they can’t do anymore. I can see my perspective on this has changed through the years and it would be good if we could qualify the issue a bit.

As I remember, at first I thought it was some terrible disability and spent the first two weeks in bed! I got mobile after that though, armed with more than a bit of anger and the attitude, “If it falls out, it falls out.”

At this point in time I can dance and leap and hike and run and swim and am having a hard time envisioning what I couldn’t do if I wanted. But I’m older now and perhaps wouldn’t want to do things a younger woman might (stripper dancing around poles and on top of tables comes to mind – lol)
